Add a template for reporting issue

I wonder if this repo should have a template to suggest to the reporter to provide some information like:

MathML Core references and suggested spec changes. Possibly mention relevant text/issues from other web standards, text from OpenType MATH / TeXBook or Microsoft Typography Issues.
Minimal HTML+MathML test case. Possibly mention existing WPT coverage.
Existing behavior with different OpenType Math font and web engines. Possibly provide screenshots or mention related bug reports on other browser or fonts trackers.

Should add what browser is being used along with which OS.

Should add which font is being used, if a font is forced or default.

bk we need to make an issue template. we tell dc what should be in the problem report.

bk likes a template that asks a couple of questions.

We should include links on where to report rendering bugs.
